Perfusion Imaging in Acute Chest Pain This study of 2475 adults with chest pain suggestive of cardiac ischemia, with a nondiagnostic EKG, found that sestamibi myocardial perfusion imaging reduced unnecessary hospitalizations without reducing appropriate admissions. Comment: perfusion imaging has been shown in several studies to be cost-effective in the triage of patients presenting to the emergency room with symptoms suggestive of cardiac ischemia. The problem of implementing such a program is that it requires cooperation between cardiologists, radiologists, and emergency department physicians. Comparison of Fatty Meal vs Cholecystokinin for Gallbladder Ejection Fraction This study compared the gallbladder ejection fractions induced by intravenous cholecystokinin versus a fatty meal in healthy subjects. The fatty meal caused a lower GBEF of 54% compared to the cholecystokinin GBEF of 76%. Comment: the response to the fatty meal was also more variable compared to the intravenous cholecystokinin. The authors state that 10 ng/kg CCK infused over 3 minutes produces a maximal GBEF but the methods used varies from clinic to clinic. The suggestion the authors give to report the GBEF as percentiles for the high fat meals may also improve reporting when using CCK.
