Preoperative Risk Stratification Using Stress Myocardial Perfusion Scintigraphy with Electrocardiographic Gating
This study found that dipyridamole nuclear SPECT scanning, the presence of diabetes, and age > 74 years were the only independent predictors of cardiac events (based on multivariate analysis) during noncardiac surgery. Comment: this study adds the new finding that segmental wall motion analysis of the SPECT data adds valuable incremental information. The authors used the Cedars-Sinai QGS program and looked at 7 segments: anterobasal, anterior, apex, inferior, posterior, septum, and lateral. Those with normal motion in all segments had a very low risk of a cardiac event at surgery. Perfusion scan findings also had an independent predictive value in addition to the segmental wall motion analysis. [ Journal of Nuclear Medicine Vol. 44 No. 3 385-390 ]
