Hormone Replacement Therapy Associated With False-Positive Treadmill Tests
This study found that postmenopausal women on hormone replacement therapy were more likely to have a false-positive exercise stress test compared to premenopausal women and postmenopausal women not on HRT. Comment: 21% of all women had a false-positive exercise stress test when compared to perfusion images. A positive stress test was defined as 1 mm or greater horizontal and/or downsloping ST depression during or just after exercise. 39% of the postmenopausal women on HRT had a false-positive exercise test. Pediatric Sedation With Melatonin
This study looked at 50 uncooperative children undergoing an MRI examination. The researchers gave the children 10 mg of melatonin (given orally, mixed into a drink) prior to imaging. After 45 minutes, 35 went to sleep. There were no complications. None of the children required a hospital, oxygen saturation monitoring, or close supervision. Comment: this novel approach appears to be both safe and effective. The children studied included 11 under the age of 4 years old. [ article ]

PET and low-dose dobutamine echocardiography in the prediction of postrevascularization improvement in left ventricular function & exercise parameters
This study found that PET was more sensitive but less specific than low-dose dobutamine echocardiography in predicting regional left ventricular function after revascularization. PET also was predictive of an improvement in exercise parameters (the rate pressure product, RPP) but echo was not. Comment: this study looked at 46 patients scheduled for CABG with an LVEF of < 50%. In this group, 19 patients showed an improvement in regional wall motion, but their LVEF did not improve. The other 27 did not show regional wall motion improvement after CABG, and their LVEF decreased. Thus, for all 46 patients, the LVEF remained the same or slightly decreased. This means that the primary improvement found was an improved RPP. Only PET findings were predictive of an improvement in the RPP. [ article ]

ECG Gated SPECT Correlates Well With Cardiac MRI
This meta-analysis compared gated SPECT with cardiac MRI and found a correlation coefficient (r) for LVEF of 0.87, for ESV r =0.92, and for EDV r =0.89. However, the variation among individual patients was fairly high. Comment: a meta-analysis is not the best way to study this issue, however, the conclusions are reasonable. If you want an accurate measurement of cardiac volumes, MRI or MUGA scanning should be done. [ J Am Coll Cardiol 2002 Jun 19;39(12):2059-68 ]
