Myocardial Bridging in a Patient With Exertional Chest Pain

This case study describes a 37 y/o man with exertional chest pain and a reversible defect of the anterior and inferoapical walls. Cardiac catheterization found normal coronary arteries during diastole but a high-grade stenosis of the mid left anterior descending coronary artery during systole. This is a "milking effect" and "stepdown-stepup" phenomenon, and is typical of myocardial bridging, which is produced by systolic compression of an epicardial coronary artery that has an abnormal intramural course. The first line therapy is beta-blockers. Surgical myotomy and stenting are reserved for cases not responding to medical treatment. PMID: 15891299
