Size of Scar Most Significant Predictor of Sustained Ventricular Arrhythmia
TITLE: Relationship of scar and ischemia to the results of programmed electrophysiological stimulation in patients with coronary artery disease

PATIENTS: 144 patients with chronic coronary artery disease who underwent electrophysiological studies (EPS) and MPI within 3 months constituted the study population.
FINDINGS: Scar, as determined by the myocardial perfusion imaging (MPI) scan, was the most significant predictor of inducibility of sustained ventricular tachycardia on EPS.
COMMENT: the presence of a large scar on MPI scanning and a low left ventricular ejection fraction are the most important predictors of spontaneous occurring or inducible sustained ventricular arrhythmias. The MPI scan is a valuable tool in determining who will benefit from an implantable cardioverter-defibrillator.
